  • How do I submit a request for a record?

    Requests can be submitted online, by email, by fax, or by mail. Please select only one method for your request - do not send the same request by multiple methods.

  • What information do I need to provide?

    An individual seeking their own record:  

    • Complete name
    • Address - in compliance with US Postal Service standards
    • Contact information - a valid telephone number or email address

    A person other than an individual seeking a record:

    • Complete name of person's agent
    • Address of person's agent - in compliance with US Postal Service standards
    • Contact information - a valid telephone number or email address
  • How do I make a request online?

    Please visit the MSP Records Resource Section for directions. Once you create your account, you will be able to submit records requests, track your submissions, and receive responses to your requests through the MSP Records Request Portal.

  • How do I make a request by fax?
